How to start enrollment:

  1. Begin by checking the SDUSD School Finder list here.

    This look-up system indicates if the household is within the school boundary area.

  2. If you live outside of our boundary area, you must apply and be accepted through the Choice program. If you have children already attending and have a younger sibling to yet enroll, you MUST contact CHOICE to apply and be accepted, in this case, it is not automatic.

  3. If you are looking for a UTK program, please check in here for an enrollment phone call to be sure all criteria is met prior to receiving or completing CHOICE and/or a school enrollment packet.

To register your child you will need:

  1. Birth certificate or Passport

  2. Vaccination record with all immunizations current

  3. Proof of residence such as a lease agreement, mortgage statement, SDGE bill, water bill or cable bill with your name and address listed

  4. School Verification (most recent report card or certificate of enrollment)

  5. IEP *if applicable 

  6. Record of Prior Special School Program *if applicable 

  7. Universal form

  8. Enrollment form*editable form on your computer

  9. Home Language Survey*editable form on your computer

  10. Health Information Exchange*editable form on your computer

  11. Proof of a dental visit after age 4 Dental Form **Only required for our UTK and Kindergarten students**

  12. Proof of a doctor check-up after age 4 Health Checkup Requirements Medical Form **Only required for our UTK and Kindergarten students**
